To sign up for Auto Replenish, simply complete and submit the form below. Once your request is received, we will evaluate your current order frequency and enter your clinic's request into our Auto Replenish system. Every month, our system evaluates whether or not you need supplies and, if so, automatically reorders them for you. All you have to do is be at the office when they arrive. It's that simple!
